You and your spouse are eligible to receive Take Charge | Live Well rewards if you are enrolled in the state's medical plan. Enrolled employees can earn up to $1,500 and enrolled spouses can earn up to $550 by completing activities to improve your health and wellness.
Wellness Rewards for July 1, 2022-June 30, 2023
Know Your Numbers and Healthy Measurements
Employees can earn up to $600 and spouses can earn up to $350.
- Complete the Health Check Survey
- Employees and spouses: $50 each
- Complete a biometric screening (Complete one of the following)
- Employees: $150, spouses: $100
- Complete one of the following:
- Visit an on-site event at a state facility or a Patient Service Center.
- Complete a Physician Form.
- Healthy weight biometrics
- Employees: $200, spouses: $100
- Complete one of the following:
- Your BMI is between18.5-29.9.
- You have a 5% weight loss from your previous benefit year's biometrics.
- Complete the reasonable alternative.
- Complete a Healthy Eating Journeys.
- Section Blood Glucose
- Employees: $200, Spouses: $100
- Complete one of the following:
- Ideal fasting glucose (less than 126).
- Improved fasting glucose (5% improvement).
- Complete a Diabetes Journey.
Additional Activities
Employees can earn up to $900 and spouses can earn up to $200.
- Complete tobacco-cessation coaching (4 coaching appointments)
- Employees and spouses: $200/completion each
- Complete a coaching appointment (1 time per month)
- Employees and spouses: $50/appointment each
- Track Healthy Habits 20 days in a month (1 time per month)
- Employees and Spouses: $50 each
- Complete a RethinkCare mindfulness program
- Employees and spouses: $50 each
- Participate in a walking or activity challenge (up to 3 times per year)

- Complete an immunization (1 time per month)

- Gym/fitness facility reward for going at least 8 times in a month (reward given 1 time per month)

- Behavioral health activity, such as the Ohio Employee Assistance Program, behavioral health therapy, SilverCloud or counseling

- Participate in community and regional activities, such as a 5K or a marathon, volunteer activities, wellness events (1 time per month)

- Participate in OPERS, Deferred Compensation or other retirement financial well-being activities or courses (1 time per month)

Rewards: How rewards will be received
Rewards earned by the state employee and his or her spouse, if applicable, will be reflected on the state employee's paycheck after the completed activity has been processed. Payroll additions will likely appear on paychecks within 8-12 weeks after the completion of an activity.
Any employee who retires or terminates prior to the paycheck that they would receive the reward on (typically up to 12 weeks) will receive a check in the mail to the address listed in OAKS, for that same pay cycle that they would have received it on. For example, if the normal cycle would have the employee receiving their reward on a Nov. 1 paycheck, but they retire in October, they will receive a check in the mail approximately on Nov. 1 for that reward.
Taxes
Rewards earned by the state employee and his or her spouse, if applicable, are taxable. Taxes are based on the amount of the reward. The taxes for rewards earned by either the employee or spouse will be deducted from the state employee's paycheck. The paycheck will reflect the amount of the rewards minus the taxes.
