Don't get caught, get your flu shot
State employees and spouses can receive a flu shot at no cost.
Get a flu shot at:
- A dedicated State of Ohio workplace
- Your health care provider (physician's office)
- An in-network pharmacy or clinic
Getting a flu shot at a state worksite or a designated Walgreens:
- Go to the Walgreens Scheduler website to schedule a flu shot at a state worksite. Times and locations for state worksites are available on the scheduler.
- Download and complete the Walgreens Vaccine Administration Record
- What to take when going to a state worksites:
- Photo ID
- Proof of insurance, such as your OptumRx card or your medical card
- Walgreens Vaccine Administration Record
Getting a flu shot at a health care provider, or an in-network pharmacy or clinic:
Employees and spouses enrolled in the state's medical plan may go to their provider or any pharmacy that accepts the state's insurance and receive a flu shot at no cost.
What to take when going to a health care provider, or an in-network pharmacy or clinic:
- Photo ID
- Proof of insurance, such as your OptumRx card or your medical card

Earn a $25 reward for getting your flu shot
State employees and their spouses who receive a flu shot may be eligible to receive a $25 reward. After receiving a flu shot:
- Visit the Virgin Pulse Hub.
- Click the Rewards tab (reward icon).
- Scroll under the Additional Activities area to the Preventive Care section and click the Immunizations link.
- Click the Take Me There button.
- Complete the online attestation form.
Members may complete multiple attestation forms and be rewarded for different actions, such as getting a flu shot and a COVID booster.
Incentive dollars earned by the state employee and his or her spouse will be reflected on the state employee's paycheck after the completed activity has been processed. Monetary incentives are taxable and taxes are based on the amount of the incentive.
