  Vision Benefits for Exempt Employees:
Two Options

If you are an exempt employee, the state pays the full cost for you and your dependents to participate in the Vision Service Plan (VSP) or Cole Managed Vision. You are eligible to enroll after one year of continuous state service. If you are a new enrollee or if you want to switch plans, you can do so using the Web enrollment site or a dental and vision change form (ADM 4720).

How the Vision Plans Work
The plans provide benefits for eye examinations, frames and lenses. When you need services, you may visit any licensed doctor. If you use a network provider, you pay only a copayment and the plan pays the rest of your costs, up to certain limits.

If you use a non-network doctor, you pay the charges for the services you receive. You submit your expenses to the plan, and you will be reimbursed a flat-dollar amount as shown in the chart below.

Contact your vision plan or visit their Web site to choose a network provider.

  20 Regardless of which vision plan you choose, benefits are payable for you and your eligible dependents for:

•   One eye examination every 12 months for those younger than age 19, or age 40
     and older;
•   One eye examination every 24 months between the ages of 19 and 39;
•   Eyeglass frames once every 24 months; and
•   Eyeglass or contact lenses once every 24 months.

 

  Dental Benefits for Exempt Employees
If you are an exempt employee, the state pays the full cost for you and your dependents to participate in the Delta Dental Plan of Ohio. You are eligible to enroll after one year of continuous state service.

How the Dental Plan Works

When you need dental care, you may receive treatment from any dentist. As shown in the chart below, the plan pays a greater percentage of your costs if you use a provider who is in the Delta network.

If you use a non-Delta dentist, the percentage of the cost the plan pays is based on the amount Delta allows for the service. If the non-Delta dentist you choose charges more, you are responsible for any cost difference — that is, you will be “balance billed.”

The Delta Networks

Delta Dental Plan of Ohio administers two dental plan options for exempt employees:

  • DeltaPreferred Option USA (DPO), in which you receive benefits whether you see Delta Preferred, Delta Premier or non-Delta providers. Your out-of-pocket expenses will be less if you use a DPO provider.
  • DeltaPremier USA, in which you receive benefits when you see any provider, but greater benefits when you use DeltaPremier providers. If you use a provider who does not participate in DeltaPremier USA, you may be balance billed.

Depending on the county in which you live or work, you may have DeltaPreferred available to you or a choice between DeltaPremier and DeltaPreferred. Please refer to the service area map
