Supplemental Life Insurance

The State of Ohio offers supplemental life insurance, through the Prudential Insurance Company of America, for exempt state employees to complement the basic (group) life insurance coverage.

If your family relies on your income, term life insurance provides the most protection per dollar spent. Term life insurance has no cash value. In deciding how much to purchase, you need to evaluate your family’s financial state and estimate how much your family would need after you have died and are no longer producing income. Include ongoing expenses such as daycare and tuition costs as well as immediate expenses at death such as medical bills and burial costs.

Eligibility

Full-time permanent employees, part-time permanent employees, judges and other elected officials serving fixed terms of office are eligible for supplemental life insurance benefits. To be eligible, you must be actively at work and performing your normal duties on the effective date of coverage.

Helpful Hint:
Evidence of insurability is provided through a form called the Health Statement Questionnaire. You must make sure the Health Statement Questionnaire is complete before sending it to Prudential for approval. You may call Prudential at (800) 778-3827.

Coverage for You

Supplemental life insurance provides coverage in increments of $10,000.

  • The minimum coverage available is $10,000. The maximum coverage available is $500,000 or six times your annual salary, whichever is less.

Coverage for Your Spouse and Children

If you are eligible for supplemental life insurance benefits, you also may choose to purchase supplemental life insurance coverage for your spouse and children.

  • You must enroll in and be covered under the supplemental life insurance plan to elect coverage for your dependents.

  • $10,000 is available for coverage of your spouse without evidence of insurability.

  • $20,000, $30,000 OR $40,000 is available for coverage of your spouse with evidence of insurability.

  • $5,000 is available for each of your children, for one monthly 72-cent deduction. Dependent children must be at least 15 days old and younger than 23 years old to be eligible. There is no requirement that dependents be enrolled in school or be dependent upon you for support.

  • The benefit amounts for coverage of your dependent(s) is only available in the event of the dependent’s death. There is no cash value or accelerated death benefit.
