You are only eligible to receive certain vision care benefits once during a specified time period. These limitations are listed below.

Examination

Once every 12 months for covered members of any age and dependent children up to age 19 (up to age 23 if student).

The availability of corrective materials (lenses and/or frames) is:

Lenses - Once every 12 months
Frames - Once every 12 months, or
Contact Lenses - In place of spectacle lenses and frames
     Necessary - Once every 12 months
     Elective - Once every 12 months
