of Ohio As a permanent State of Ohio employee, you and your dependents are eligible to receive dental and vision benefits after one year of continued state service. It is important for you to understand what benefits you are eligible to receive, how they work and how to apply for them. Carefully reviewing this booklet will increase your understanding of your benefits. It provides summaries of the dental and vision benefit programs available to you. All the information in this booklet is for exempt employees only. Bargaining unit employees should consult the Union Benefits Trust at (800) 228-5088 for information. Although most of the information you need regarding these benefits is in this booklet, there may be times when you need additional information or clarification about your coverage or about dental and vision practices and procedures. If you have any questions regarding your dental or vision benefits, you can call the payroll/personnel officer at your agency, Delta Dental at 1-800-524-0149, Vision Service Plan at 1-800-877-7195, Eye Med Vision Care 1-866-723-0514 or Benefits Administration Customer Service at 1-800-409-1205.
