Custom Enterprises, Inc., the fulfillment house used to provide agencies with employee benefit handbooks throughout the year, has changed its location. Therefore the form used to order handbooks has been revised to reflect their new address and phone numbers. The form is linked here for your use. The Health Care & Long Term Care booklet (purple) will be reprinted and distributed to agencies within the next few weeks. You will be provided with one for each of your employees using the employee count on payroll records, plus 10 percent extra for your use throughout the year. We will alert you by Fax Bulletin of this distribution. An additional quantity will be shipped to Custom Enterprises at the time they are distributed to agencies to accommodate any other needs.
