Open Enrollment will be May 2 through May 13, 2005 for an effective date of July 1, 2005.

We will be sending Open Enrollment Comparison Chart and Guides based on the employee numbers you return to our office. We will be sending an additional 10% extra of the Open Enrollment Comparison Chart and Guide for your use during the next 12 months so please provide us with only the total current number of employees in your agency.

If you require distribution of Comparison and Guides to a location(s) other then the one listed below, please send the following information: the name of a person at each location, their phone number, address and number of employees on a separate paper.
