As we continue to strive to meet the needs of our customers, we are in the process of discontinuing the use of Fax Press. We will now communicate information to you via email or fax, if email is not an option. To ensure that we communicate all necessary information to the appropriate parties we are updating our Agency Contact List for benefits contacts, payroll, disability and worker’s compensation personnel. In order to update our list we are asking that you please complete the information requested at the bottom of this page and return the form by January 11, 2005 by fax to 614-466-0831. It is essential that we receive a contact for all areas listed below in order to assist you properly.
