If you need to
file additional
information,
take your letter from the TPA to your attending physician so
that he/she may see exactly what information is requested.
Additional Information

If there is not enough information to approve your claim, you may receive a letter requesting additional information on your claim from the TPA or your agency. Any additional information should be submitted to your personnel officer.

Your personnel officer will forward your additional information to either CompManagement or your agency's director for reconsideration of your claim.
