Handling Your Claims

If you are a member of OSTA or FOP2, your claim will be approved or denied by your agency.

If you are a member of OCSEA/AFSCME or 1199, your claim will be approved or denied by CompManagement on behalf of the Department of Administrative Services.

Filing

You must file an Accident or Illness Report (Form ADM 4303) and an Occupational Injury Leave Application (Form ADM 4743). Return both forms to your personnel officer.

You will need to file for Occupational Injury Leave (OIL) before a deadline specified by your union contract. You can find your bargaining unit's filing deadline in your union contract.

You must also have your attending physician complete the Occupational Injury Leave Benefits Attending Physician Statement (Initial Treatment) (Form ADM 4721). Filing this form is not subject to the deadlines above. However, filing the form earlier will help get your OIL benefits approved quickly.

File for both OIL and workers' compensation at the same time. If your workers' compensation claim is approved, your MCO will pay for any approved medical expenses. OIL pays for any lost time as a result of an approved injury in place of workers' compensation benefits.
