Injuries, Illnesses and Conditions Considered for Disability Leave Benefits
 
The following conditions may be considered for disability leave benefits:
  1. Any non work-related injury or illness that your doctor predicts will keep you from performing your job duties for more than 14 days

  2. Any mental health or substance abuse condition for which all the following are true:

    • You are receiving ongoing treatment

    • You are referred by the Employee Assistance Program (EAP) to a licensed professional or already are receiving treatment from a licensed mental health provider or a certified drug treatment professional

    • Your health care provider determines that the condition or its treatment prevents you from performing your job duties for more than 14 days

  3. Childbirth in which recovery lasts longer than the six weeks covered by childbirth/adoption leave
