| Rules of Filing
If you are filing for disability leave benefits, you are responsible
for all of the following:
- You must file all applications, forms and other information regarding
your claim with your agency's personnel office.
- You are responsible for informing your agency or the Department
of Administrative Services (DAS) of any change in your mailing address.
- You are responsible for providing medical documentation to substantiate
your disabling condition. A lack of substantiating documentation may
result in the denial of your claim.
- Check your attending physician's portion of the Application for Disability
Leave Benefits to make sure that he/she has completely supplied all
the necessary information on the form.
