Vehicle and Watercraft To Surplus Information Sheets

IMPORTANT INSTRUCTIONS:

The Vehicle To Surplus Information Sheet (ADM 3911) and Watercraft to Surplus Information Sheet (ADM 3912) is now available in a digital file format.

The Printable Document File (.PDF) format is available to download and complete using Adobe's free Acrobat Reader. Should you have the Adobe Suite you can save the completed form to your hard drive.

The Electronic Document Format (PDF) version of the ADM 3911 and ADM 3912 can be completed and sent electronicly to State Surplus. When typing your name in the Releasing Authority's Name block it is considered an authorizing signature to release item to State Surplus.

When submitting this form to State Surplus we asked that you print the completed form on three (3) white copies. Then obtain the appropriate signatures at your agency, and forward them to Surplus along with the property you are turning in for processing. The approved copy will be returned to your agency for your records. This eliminates the need for handwriting, storing forms and makes the documents more legible.
