Position Audit Request

  • Any classified county employee who is not currently on probation can request a position audit to be conducted.
  • A Position Audit Request Form (Microsoft Word form) must be completed and returned to the County Services Unit (CSU) either by email or regular mail. It is important to complete this form accurately and in its entirety. Please email the completed form as an attachment to Sharon.Stevens@das.state.oh.us. or mail to: Ohio Department of Administrative Services, County Services Unit, 30 E. Broad St. 28th Floor, Columbus, Ohio 43215-3414. Attn; Sharon Stevens, County Services Supervisor.
  • Once the CSU has received the request and verified that the employee is a classified employee, a Position Audit Questionnaire (PAQ) will be emailed to employee.
  • The employee then has thirty days to complete the PAQ in its entirety and return it to the CSU. The employee may also send any additional attachments that they feel will help justify their reclassification.
  • A PAQ will also be sent to the employee's human resources agency representative to be forwarded to the employee's immediate supervisor. The agency representative will then forward the PAQ via email to the CSU within thirty days along with an updated position description that has been approved in the last six months.
  • Once both PAQs have been submitted to the CSU, the PAQs and attachments will be assigned to a human resources analyst who will then review the forms and may contact the employee and agency for additional information. The analysts findings, which will be typed in a report, will be reviewed by the CSU job audit coordinator for approval.
