SUBJECT: CHILD CARE VOUCHER APPLICATIONS

The Child Care Voucher Program (CCV) application period begins on January 21 and ends on April 15, 2005. This program provides cash grants to eligible employees who had work-related child care expenses in 2004.

Employees will receive notification of the application period and the specifics of the program both in the next edition of Pathways and via a payroll insert or by email. BAS will not be mailing a supply of CCV applications to agencies this year. If your agency participates in the child care voucher program, please make copies of the application on pages 2 and 3 of this memo for your employees who may request it.

Please note that agencies must complete Section B of the application before it may be submitted to BAS for review. Employees are instructed to submit their completed form with a copy of their US income tax return before the deadline of April 15. Please ensure that employees meet the eligibility requirements and have completed the application in its entirety before submitting them to BAS. Applications not received by BAS or postmarked by April 15 will not be processed.

As a reminder the basic eligibility requirements are:

  1. You must have been employed by the State of Ohio for at least part of both 2004 and 2005.
  2. Your family adjusted gross income for 2004 must be under the limit set by bargaining agreements.
  3. You must have been an exempt employee, represented by either OCSEA or District 1199, or be an eligible employee of the Attorney General’s Office.
  4. You must have been a permanent full-time or permanent part-time employee in 2004.
  5. Your work-related child care expenses must have been for children who were younger than 13 in 2004.
  6. The expenses must have been equal to or more than the maximum benefit for which you are eligible
