The Child Care Voucher Program (CCV) application period begins on January 21 and ends on April 15, 2005. This program provides cash grants to eligible employees who had work-related child care expenses in 2004. Employees will receive notification of the application period and the specifics of the program both in the next edition of Pathways and via a payroll insert or by email. BAS will not be mailing a supply of CCV applications to agencies this year. If your agency participates in the child care voucher program, please make copies of the application on pages 2 and 3 of this memo for your employees who may request it. Please note that agencies must complete Section B of the application before it may be submitted to BAS for review. Employees are instructed to submit their completed form with a copy of their US income tax return before the deadline of April 15. Please ensure that employees meet the eligibility requirements and have completed the application in its entirety before submitting them to BAS. Applications not received by BAS or postmarked by April 15 will not be processed. As a reminder the basic eligibility requirements are:
