Interested in earning $25?
Participate in a free health screening at one of the upcoming Take Charge! Live Well! Road Show Events
You may be asking yourself what's a Road Show Event...this is an opportunity for employees and spouses enrolled in one of the state's health plans to learn more about Take Charge! Live Well! and to participate in a free work site health screening. Members of the DAS Benefits Administration Services Team will hit the road and travel to certain work site locations across the state to provide more information and answer questions about this program. Additionally, the three Take Charge! Live Well! providers will be available to conduct health screenings of blood pressure, blood glucose, total cholesterol, HDL and Body Mass Index.

Road Show Key Points
Earn $25. You can earn $25 of your $100 Take Charge! Live Well! incentive by participating in the health screenings.
Open to enrolled employees & spouses. Road show events are available only to employees and spouses who are enrolled in one of the state's health plans.
Road Show events are open. Road Show events are open to employees who work in the facility, as well as employees from other state facilities. Check with your agency about attending an event at a neighboring state facility.
Bring your insurance card. Your ticket in for the free health screening is your health insurance card from Aetna, OhioMed, Paramount, The Health Plan or UnitedHealthcare. |
Take Charge! Live Well! Incentive Awards Hit Paychecks
Take Charge! Live Well! incentives will continue to be added to the last paycheck of the month approximately 8 to 12 weeks after a TCLW activity has been completed. Incentives are noted with "TCLW $$$$." The March incentive rewards include completion of health assessments, health screenings and some online lifestyle behavior change programs completion.
Future incentive payments will be included in paychecks on the following dates:

Take Charge! Live Well! is the state's new health and wellness initiative designed to improve the overall health of employees and their families. The program starts with a health assessment that provides an easy-to-understand view of your current health. In addition to the health assessment, Take Charge! Live Well! offers a variety of programs geared towards a healthy lifestyle.
Employees enrolled in one of the state's health plans are eligible to participate. Enrolled spouses are also eligible.
Take Charge! Live Well! features a financial incentive program that can result in up to $200. You will receive $50 for taking the health risk assessment. For each health improvement program you participate in, you will receive an additional $25, up to $50.
If your enrolled spouse participates, you could earn an extra $100.
