BUILDING ID BADGE
Building identification (ID) badges will be issued to all employees and contractors working in state-owned buildings managed by the Ohio Department of Administrative Services (DAS), Office of Properties and Facilities. Building IDs will be used to identify state employees as well as to permit after-hours access when approved by the employee’s agency. Building ID badges are to be worn by tenants/employees at all times in such a manner that they are readily identifiable.

New and Replacement Building ID Badge:
New badge: Complete the building ID request form. Print form and obtain manager’s signature. Bring completed form to the DAS Building Manager’s Office.
Replacement badge: Complete the attached form including the “reason for replacement badge” field. Print form and obtain manager’s signature. Bring completed form and broken/malfunctioning ID badge to the DAS Facility Manager’s Office.
Contractors whose assigned work location will be in a state-owned building managed by Ohio DAS and whose assignment may continue for more than a few weeks, should receive and display a building ID. Complete the attached form. On the form, in the field for Employee ID, enter “contractor.”
Please follow the state employee instructions above for both new and replacement badges. Print form and obtain manager’s signature. Bring completed form to the DAS Facility Manager’s Office.
Photos for ID cards are taken every Monday through Friday 8 a.m. to 4 p.m. in the DAS Facility Manager’s Office. No appointment necessary.
Termination of Building ID Badge:
If a state employee or contractor terminates employment with an agency within a state-owned building, the badge holder must surrender his/her building ID badge to the appropriate manager within the agency. Agencies are responsible for returning all badges to the DAS Building Manager’s Office. The Building Manager’s Office will process the terminated card by: 1) deactivating the employee/contractor’s access to proximity readers on agency floors and 2) by destroying the ID badge.
If a state employee or contractor transfers to another state agency, the employee’s/contractor’s current building ID badge must be surrendered to the appropriate manager within the transferring agency. The transferring agency must return the badge to the DAS Building Manager’s Office for destruction. The receiving agency must seek a new building ID by completing and signing the attached Building ID request form.
DESTRUCTION OF ID BADGES:
Agencies must return all surrendered badges to the DAS Building Manager’s Office for proper destruction
